NEW! Orthodontic Benefit
Orthodontic services are reimbursed according to a fee schedule up to a lifetime maximum of $1,700 per eligible child. A period of orthodontic treatment starts on the first day your dependent child incurs a covered expense for orthodontia and extends for a period of 24 consecutive months or less if the treatment is completed in less time. This benefit is not included in your annual maximum.

* Diagnosis and insertion of the initial appliance: Once per lifetime up to $500.

* Active monthly visits: Up to $50 per visit with a maximum of 24 consecutive months.


Participating Dentist
If you use a participating dentist you DO NOT need to submit any paperwork. The participating dentist is fully responsible for submitting all invoices to Cook Associates. A list of participating dentists was provided at the September UTS meeting. Contact your Trust Fund Rep. if you need a list of participating dentists in our program.

Non-Participating Dentist
If you prefer to use your own dentist, you MUST pay the dentist first. The Trust Fund will reimburse you directly. You can submit your bill in one of two ways:

1) Your dentist can send an invoice of services rendered and the amount billed electronically or by U.S. Mail to:

2) Obtain a printout of services rendered and amounts paid, and send it yourself to the address above. Please indicate your name, address, telephone number and SEAFORD TRUST FUND on a separate piece of paper attached to your receipts. (*Submitting dental forms to the Trust Fund Rep. delays the payment.)

* If you choose to use a participating dentist, you DO NOT need to submit any paperwork. The participating dentist is fully responsible for all invoices and will supply printouts to Cook Associates.

(ALL claims for reimbursement should be sent directly to Cook Associates. Be sure to keep the originals in case you need to reference a claim. Please be patient for your payment.)

*** Remember, there is a $2,000 maximum Dental Reimbursement per calendar year.
