General Vision
You may use General Vision for your eye exam and glasses. You can receive a vision test and up to $75 for frames. General Vision offers frames that retail up to $120 in addition to the vision test. Please contact your Trust Fund Rep. for a voucher.

Other Vendors
UTS members are entitled up to a $200 reimbursement (after submitting a receipt of purchase) each calendar year. An updated provider / benefit booklet was distributed at the September UTS meeting. Contact your Trust Fund Rep. if you need an updated booklet.

Reminder: You DO NOT need eyeglass forms. All you have to do is go to GVS and give your name, social security number, our local number (#1780) and our school number (#6520). It is also helpful if you carry your NYSUT card for verification. If there is a problem at the store, have them call (800)-VISION-1.

Contact Lenses
Disposable contact lenses are NOT available to spouse or independents. Extended wear for spouse and independents is allotted the same 30% discount. MEMBERS ONLY can purchase disposable lenses, but they must be purchased for ONE year at a discount of $120. They can also receive $5 off eye exams.

Soon you will receive a coupon to utilize services at GVS. The brochure is good for you, your spouse or independent. If you do not use the coupon in the brochure, then your spouse or independent is only entitled to 30% off. You MUST be present with your spouse or independent to receive the discount.
